How it Works
Only select the service lines that you’ve directly interacted in 2018 and comment on the services you received during that time. Each service line takes approximately three minutes to evaluate.
As a token of our appreciation, upon completion of the survey, you will receive a barcode to redeem your choice of a UC Davis T-shirt, mug or bag, as well as a coupon for the UC Davis Stores.
